Fees and insurance
$150 for 50-minute individual or family session
$200 for 1 hour professional consultation
I am an out-of-network provider with most insurance carriers. This means that I do not make claims directly to your insurance company. However, I can provide you with a bill that you may submit to your insurance company to make a claim for reimbursement. Questions to ask your insurance company before we begin:
Are there out-of-network benefits for this policy?
Do I have a mental or behavioral health policy with out-of-network benefits?
What are the requirements to use out-of-network benefits?
Is prior authorization required?
Is a referral required from my primary care physician?
Do I have an out-of-network deductible?
If yes:
What is my out-of-network deductible? How much of my out-of-network deductible has been met?
What is the start date of the calendar year my out-of-network policy is based on?
Is there a session limit?
If yes:
What is the session limit?
How many sessions do I have left?
What percentage of services is covered/what is my co-insurance?
Policies
Payment is due at the beginning of each session and can be made by cash or check. I require at least 24 hours notice for appointment cancellations. Missed appointments or cancellations without this notice will be charged the full session fee. I ask that parents remain in the office waiting area while their children are in session unless we have specifically agreed to an alternative.
